Primary Care Memberships
Primary care memberships are available for adults 18 and older. Membership includes:
Extended, unhurried office visits.
Same or next day acute care office visits.
Direct-to-physician phone/text access.
Optional telemedicine visits.
Old-fashioned house calls when indicated (limited radius).
Free in-office lab testing (e.g., urinalysis, strep, flu).
Access to discounted imaging, medications, and out-of-office lab testing.
Pricing as of 1 June 2023.
We charge a one-time Registration Fee of $100.
Group discounts for small businesses available.
Pricing includes all services and procedures provided at ColGACare. Depending on your needs, additional testing may be recommended (eg: an x-ray or send-out lab test). We believe in complete transparency and will happily discuss the reason a test is being recommended, the risks, benefits, and alternatives to the test, as well as the price of the test and options for payment. We will do everything we can to make excellent care affordable for you.

Acute Care Appointments
Acute care for ColGACare member adults is included as part of your membership. While we don't offer pediatric memberships at this time, we are happy to provide discounted acute care appointments for the minor children of our members, on a case-by-case basis. We will always let you know ahead of time if we think your loved ones needs would be better, or more safely, served by a Board-certified Pediatrician or a dedicated Pediatric Emergency Department.

*Our members are always our priority. Acute Care appointments for non-members are subject to availability and at the discretion of Dr. G. Please remember, ColGACare is NOT a walk-in clinic. You must call ahead for an appointment.

There is an additional $25 charge for each additional child evaluated at the same visit.
