Primary Care Memberships

Primary care memberships are available for adults 18 and older. Membership includes:

Extended, unhurried office visits.
Same or next day appointments often available.
Direct-to-physician text access & optional telehealth.
Personalized care plans.
Access to discounted labs & cash prices for imaging.
In-house medication dispensing.

We charge a one-time Registration Fee of $150.

Group discounts for small businesses available.

Acute Care Appointments

Acute care for ColGACare member adults is included as part of your membership. While we don't offer pediatric memberships at this time, we are happy to provide discounted acute care appointments for the minor children of our members, on a case-by-case basis. We will always let you know ahead of time if we think your loved one's needs would be better, or more safely, served by a Board-certified Pediatrician or a dedicated Pediatric Emergency Department.

Pricing includes all services and procedures provided at ColGACare. Depending on your needs, additional testing may be recommended (e.g., x-ray or send-out lab test). We believe in complete transparency and will happily discuss the reason a test is being recommended, the risks, benefits, and alternatives to the test, as well as the price of the test and options for payment. We will do everything we can to make excellent care affordable for you.

At ColGACare, we are dedicated to transparent pricing. If you have any questions about what any of these mean, or why they are necessary, please, don't hesitate to ask!

Overhead factor for meds: 20%
(This covers the cost of labels, packaging, bottles, shipping, and disposal of expired meds.)

Overhead factor for labs: 15%
(This covers the cost of labels, other supplies, in-house processing, and certain add-on tests.)
There are rare exceptions to the percentage-based factor in pricing certain labs. For example, "Urine Studies," when sent out, are a flat $30.00. This is because we generally don't think it's fair to charge patients different amounts based on the RESULTS of their tests, which frequently trigger more follow up tests. By charging everyone a "flat $30," it is a fair and predictable price for all of you, and it "all comes out in the wash" for us.

Patient fee for shipping supplies: 1.50
(This covers the physical supplies, the shipping fee itself is calculated by weight and zip code.)

Missed Appointment Fee (NEW as of 2026!): 25.00
(If you are more than 10 min late to an appointment without contacting us, you may be considered a 'NO SHOW' for that appointment. If you are more than 20 min late to an appointment, under any circumstances, you may be considered a 'NO SHOW' for that appointment. Repeated "NO SHOWS" may be cause for contract termination.)
